Preparation and issuance of a permanent residence permit

1. a passport document of a foreigner or an identity document without, with or without a type D visa, unless otherwise provided by the legislation and international treaties of Ukraine;
2. a document proving the identity of the legal representative and a document confirming the powers of the person as a legal representative, if the documents are submitted by the legal representative;
3. translation into Ukrainian of a page of a foreigner`s passport or stateless person`s identity document with personal data, certified in accordance with the procedure established by law;
4. a document confirming the payment of the administrative fee or a document on exemption from its payment;
5. a copy of the resolution on granting an immigration permit or, according to the category of foreigners and stateless persons, a copy of the decision on granting an immigration permit:

• by foreigners or stateless persons who arrived in Ukraine for permanent residence before the entry into force of the Law of Ukraine «On Immigration» and have a mark of residence in the passport of a citizen of the former USSR of 1974 or have received a permanent residence permit, the original and a copy of the passport of a citizen of the former USSR of 1974 model with a mark of residence or a permanent residence permit in Ukraine, the original and a copy of the passport of a citizen of the former USSR of 1974 model with a mark of residence or a valid residence permit;
• by foreigners or stateless persons who were forced to leave their places of permanent residence in the Autonomous Republic of Abkhazia of the Republic of Georgia and arrived in Ukraine, as well as their adult children who arrived in Ukraine with them before reaching the age of majority, — a temporary certificate (or a certificate confirming the fact of arrival of a minor with his/her parents) obtained after arrival in Ukraine, and a document confirming the fact of residence in Ukraine for at least five years issued by the territorial bodies of the SMS;
• by foreigners or stateless persons who arrived in Ukraine as orphans in connection with armed conflicts in their places of permanent residence and are being raised or were raised in state children`s institutions or family-type children`s homes or who are under the guardianship or custody of Ukrainian citizens, a document confirming upbringing in a state children`s institution or family-type children`s home or the establishment of guardianship or custody.
• foreigners or stateless persons who, prior to the decision to terminate their Ukrainian citizenship, permanently resided in the territory of Ukraine and, after making such a decision, remained permanently resident in its territory, shall submit a certificate of termination of Ukrainian citizenship of the established form issued by the territorial body/territorial subdivision of the SMS.

A foreigner or stateless person, when submitting documents for a residence permit, shall submit to an employee of the territorial body/territorial subdivision of the SMS, an authorised entity, the originals of the documents specified in subparagraphs 1, 2 and 5 of this paragraph.

The application form shall be accompanied by the originals of the document referred to in subparagraph 3 of this paragraph and the document confirming the payment of the administrative fee, and copies of the documents referred to in subparagraphs 1, 2 and 4 of this paragraph and the document on exemption from the administrative fee, certified by an employee of the territorial body/territorial subdivision of the SMS, authorised entity by marking «According to the original» and signing with his/her position, surname, initials and date.

The originals of the documents referred to in subparagraphs 1 and 2 of this paragraph and the document on exemption from payment of the administrative fee shall be returned to the foreigner or stateless person. Instead of the documents referred to in paragraphs 2, 4, 6 of clause 32 of the Procedure, a person recognised as a stateless person in accordance with Article 6 of the Law of Ukraine «On the Legal Status of Foreigners and Stateless Persons» shall submit a duly executed decision on recognition as a stateless person and a receipt of payment of the state fee or a document on exemption from its payment (the original shall be returned upon presentation, a copy shall be submitted).
