To do this, you need to upload your written appeal as a scanned or photocopy. Requirements for a written appeal sent using the Internet, electronic communication means ("e-appeal"), according to the Law of Ukraine "On Citizens' Appeals", Article 5:

• surname, name, patronymic of the applicant
• full address of the place of residence
• the essence of the issue raised, the e-mail address to which the response can be sent or other means of communication
• date and signature of the applicant.

Please note! A written appeal without indication of the place of residence, not signed by the author(s), as well as one from which it is impossible to establish authorship, is recognised as anonymous and is not subject to consideration. (Law of Ukraine "On Citizens' Appeals", Article 8).
