State registration of an individual as an entrepreneur

1. An application for state registration of an individual as an entrepreneur, which may include a request to register such a person as a value added tax payer of services and/or to choose the simplified taxation system;
2. notarised written consent of parents (adoptive parents) or guardian or guardianship authority — for an individual who has reached the age of sixteen and wishes to engage in entrepreneurial activity but does not have full civil capacity.
3. an agreement (declaration) on the establishment of a family farm — in case of state registration of an individual who establishes a family farm on his/her own or with family members in accordance with the Law of Ukraine «On Farming».

If the documents are submitted in person, the applicant must present a document that certifies his or her identity in accordance with the law.

If the documents are submitted by a representative, a copy of the original (notarised copy) of the document confirming his/her powers shall be additionally submitted.

For the purposes of registration actions, a document certifying the powers of a representative may be:

1. a notarised power of attorney;
2. a power of attorney issued in accordance with the laws of a foreign country.
