List of documents required to receive an administrative service, the procedure and method of their submission, and, if necessary, information on the conditions or grounds for receiving an administrative service

1. An application addressed to the head of the district administration / director of the department of municipal administrative service, agglomeration (Annexes 1, 2 to this information procedure and method of their card).

2. An identity document:

  • Passport of a citizen of Ukraine (for citizens of Ukraine)
  • Birth certificate — for children under the age of 14. If the birth certificate of a child under the age of 14 indicates that the child`s parents or one of the parents is a foreigner or stateless person, a certificate of registration as a citizen of Ukraine is additionally submitted for the first time to register the child`s place of residence.
  • Temporary certificate of a citizen of Ukraine. For other persons (foreigners, stateless persons), one of the following documents:
  • Permanent residence permit.
  • Temporary residence permit.
  • Refugee certificate.
  • Certificate of a person in need of complementary protection.
  • Certificate of a person granted temporary protection.

In case when submitting documents, a foreigner or a stateless person establishes that they applied for registration of their place of residence after 30 calendar days from the date of receipt of a permanent residence permit, temporary residence permit, deregistration of the previous place of residence or submitted an invalid document for registration of a new place of residence, the foreigner or stateless person is referred to the territorial unit of the State Migration Service of Ukraine in accordance with the territory of service to take administrative action or other measures specified by law.

3. A document confirming the payment of the administrative fee (in case of registration of the place of residence simultaneously with the removal from the previous place of residence, the administrative fee is charged only for one service).

4. Documents confirming:

  • The right to live in housing, in particular, a certificate of ownership, a warrant, a lease (rental, sublease) agreement, a dormitory rental agreement (for students), a court decision that has entered into force granting a person the right to move into a dwelling, recognition of the person`s right to use or ownership of a dwelling, the right to register a place of residence, or other supporting documents. In the absence of these documents, the registration of a person`s place of residence (stay) is carried out with the consent of the owner (co-owners) of the dwelling, the tenant and his family members, the authorised person of the dwelling (these documents and consent are not required when registering the place of residence (stay) of minors and minor children at the address of the declared/registered place of residence (stay) of their parents or legal representatives, or one of them. In case of a mortgage or trust property as a way to secure fulfilment of obligations, a written consent of the relevant mortgagee or trustee is additionally required for registration of a person`s place of residence.
  • The right to stay or be registered in a specialised social institution or other provider of residential social services (a certificate of acceptance for service in a specialised social institution or other provider of residential social services of the established form, a copy of the certificate of registration of a homeless person, the form of which is approved by the Ministry of Social Policy of Ukraine (for persons registered in such institutions or establishments).
  • In the case of placement of an orphan or a child deprived of parental care in an institution for orphans and children deprived of parental care, a family-type children`s home, or a foster family, the place of residence of such a child is registered at the address of the institution, family-type children`s home, or foster family`s residence. The submission of an application for registration of the place of residence or change of place of residence of an orphan or a child deprived of parental care is the responsibility of the guardianship and custody authorities at the place of residence of such a child. In addition, for other persons, the following applications are required.

5. Military registration document (for citizens of Ukraine who are subject to military registration or are on military registration).

6. If the registration of the place of residence is carried out simultaneously with the removal from the previous place of residence (stay), a separate application for removal from the declared/registered place of residence is not submitted.

7. If the application is submitted by the legal representative of the person, in addition to the above documents, the following documents shall be submitted:

  • Identity document of the legal representative (representative).
  • A document confirming the authority of the person as a representative, except when the application is submitted by the legal representative of a minor child.

8. Persons who have reached the age of 14 submit an application for registration of their place of residence on their own.

9. Registration number of the taxpayer`s account card (optional for updating data in the Register of L’viv City Territorial Community).

If a person under the age of 18 submits an application for registration of a place of residence, registration of the place of residence is carried out with the consent of the parents or other legal representatives of such person. Consent is not required if the person is an education student and registers his/her place of residence in a dormitory belonging to the management of an educational institution. An application for registration of the place of residence of a person under the age of 14 is submitted by one of his/her parents or other legal representatives with the consent of the other parent or legal representative (unless the child`s place of residence is determined by a relevant court decision or decision of the guardianship and custody authority). The consent of the parents or other legal representatives may be given in the presence of the person receiving the application for registration of the place of residence or notarised in accordance with the procedure established by law.

If the place of residence of the parents or other legal representatives of a person under the age of 18 is declared/registered at the same address, the consent of the other parent or legal representative is not required.
